In partnership with Neighborhood Family Practice, a COVID-19 Vaccine Clinic will be held on April 9, 2021, from 8:30 am - 12:00 noon at the Community Center for the Deaf and Hard of Hearing at 11635 Euclid Ave., Cleveland, Ohio 44106.

• ASL Interpreters available
• Schedule an appointment at: http://bit.ly/CHSCvaccine  
• Must be 18 years of age and older.
• There is no cost for the vaccine.
• No health insurance information will be collected.
• US citizenship is NOT required for the vaccine.
• You will receive an email confirmation after you register (if email is
provided).
• Please bring a photo ID with you to the appointment. It does not have to
be a government-issued photo ID.
